Welcome to Haycock Elementary School!

We're excited to welcome our newest Cougars! 

To join us, first determine if your student lives in Haycock Elementary School boundary by viewing the FCPS boundary locator. If your student does live in our boundary, please fill out the new student registration signup/intake form below. Your form will be submitted to our school registrar, who will be able to assist you with the registration process.

Our student registration process involves three steps:

  1. Complete the Enrollment Packet
  2. Gather Required Materials
  3. Make an Appointment

As you go through the registration process, our school registrar is available and ready to help answer your questions and provide you with assistance.

Gathering the Required Materials

Along with the enrollment packet, supplemental documentation must be provided for Proof of Identity, Proof of Residency, and the School Health Entrance Form.

Proof of Identity documents for both the enrolling parent and the student are required to be originals. These will be verified and copied at your registration appointment. The School Health Entrance is required to be filled out by a medical professional and reflect, either in the form or attached, a physical examination conducted within one year of enrollment and an up to date immunization record as described here.

Accepted Documents:

  • Original birth certificate: If a birth certificate is not available, the FCPS affidavit with substantiating documentation is required.     
  • Parent Photo ID: Only a parent who is named on the child's birth certificate is eligible to enroll the student. The parent will be required to present government-issued photo identification (driver's license, passport, etc.)
    • Military IDs cannot be accepted as they cannot be photo copied by school personnel.
  • Proof of residency: A current, fully-executed lease (signed by both the tenant and landlord/management company), the deed/title/closing documents to the home is required, or a current mortgage statement. If you cannot provide one of these documents, you must complete and have notarized two FCPS residency affidavit forms and provide the necessary documentation at registration.

Make an Appointment

Registration appointments will be available starting July 19th. These will be 30 minutes long to go over your enrollment packet and documentation. Parents are not allowed in the building without an appointment and masks are required. Please ring the front bell at Door 1 when you arrive.

What to Bring:

  • Completed and printed enrollment packet
    • Some forms in the packet are 2 sided while others are not. Be sure to be careful when printing.
  • Original student birth ceritificate and parent photo ID
  • Proof of residency in the form of a deed or a lease

Are You Registering a Student Whose Home Language Is Other Than or In Addition to English?

Student families wishing to register a student with a home language other than or in addition to English should please contact the FCPS Student Registration Welcome Center. Here professionals may assess your student's English language skill and/or educational level. These assessments can provide valuable information to the local school and support your child's instructional needs.  

Do You Have Special Registration Situations? 

Special registration situations can include nonparent registrations, situations involving divorced or separated parents, homelessness, foster care, and foreign exchange students.
