Welcome to Haycock Elementary School!

We're excited to welcome our newest Cougars! 

First determine if your student lives in our school boundary by viewing the FCPS boundary locator. If your student does live in our boundary, please follow the 3 step registration process.

Our student registration process involves three steps:

  1. Complete the Enrollment Packet
  2. Gather Required Materials
  3. Make an Appointment

Registration Forms and Enrollment Packet

General Registration Requirements:

  • View and complete the Enrollment Packet of FCPS registration forms.
  • View and prepare the FCPS minimum immunization requirements.
    • The School Health Entrance Form, a part of the enrollment packet, is required to be filled out by a medical professional and reflect, either in the form or attached, a physical examination conducted within one year of enrollment and an up to date immunization record as described in the link above.

Kindergarten Registration Requirements:

  • View and prepare the above packet and immunizations.
  • View and complete the Pre-Kindergarten experience form.
    • Please be sure to check both sets of boxes and use the last form of preschool the child received (possibly pre-Covid).
  • Ensure your student is the appropriate age and meets the requirements for Kindergarten by visiting the County kindergarten registration page to learn more. This page has videos as well as the five-step process for enrolling a new student.

Gathering the Required Materials

Along with the enrollment packet, supplemental documentation must be provided for Proof of Identity and Proof of Residency.

Proof of Identity documents for both the enrolling parent and the student are required to be originals. These will be verified and copied at your registration appointment.

Please prepare one of each of the 3 following documents:

  • Original birth certificate: 
    • US state birth certificate
    • Consular Report of Birth Abroad
    • Foreign birth certificate in English
    • Foreign birth certificate with notarized official English translation
    • If an original birth certificate or above named document is not available, the FCPS affidavit with substantiating documentation is required. 
      • Affidavit forms may be obtained at the school or from this link.
  • Parent Photo ID: 
    • Government issued photo ID, eg. driver's license, passport, etc.
      • Military IDs cannot be accepted as they cannot be photo copied by school personnel.
      • Only a parent who is named on the child's birth certificate is eligible to enroll the student.
  • Proof of residency: 
    • A current, fully-executed lease (signed by both the tenant and landlord/management company)
    • The deed/title/closing documents to the home is required, or a current mortgage statement. 
    • If you cannot provide one of these documents, you must complete and have notarized two FCPS residency affidavit forms and provide the necessary documentation at registration.
      • Affidavit forms may be obtained at the school or from this link

Make an Appointment

Registration appointments are available to be made at any time, with registration for the next school year opening in February. These will be 30 minutes long to go over your enrollment packet and documentation. Parents are not allowed in the building without an appointment and masks are required. Please ring the front bell at Door 1 when you arrive.

What to Bring:

  • Completed and printed enrollment packet
    • Some forms in the packet are 2 sided while others are not. Be sure to be careful when printing.
  • Original student birth certificate and parent photo ID
  • Proof of residency in the form of a deed or a lease.

Are You Registering a Student Whose Last School Was Outside the US or Do You Need Language Assistance for Your Registration?

Student families wishing to register a student who has never attended a US school should please contact the FCPS Student Registration Welcome Center. Here professionals may assess your student's English language skill and/or educational level. These assessments can provide valuable information to the local school and support your child's instructional needs.  

Student families who may have difficulties conducting a registration appointment in English and may need language assistance can also contact the FCPS Student Registration Welcome Center to complete the registration. 

Do You Have Special Registration Situations? 

Special registration situations can include non-parent registrations, situations involving divorced or separated parents, homelessness, foster care, and foreign exchange students.

Military Family Registration 

FCPS welcomes responsive and collaborative communication and partnerships with the parents and families of our students.
